This is to notify all staff that 2016 staff Audit form will be available on-line with effect from Monday 7th December, 2015. All members of staff are advised to fill the form and act accordingly.
NOTE: attach a copy of your December 2015 salary pay slip. Endeavor to return the completed form to the Internal Audit Unit not later than 11th January 2016
Failure to return the form as requested above could result to non-payment of salary for the month of January, 2016 to those concerned
False certification will not be condoned.
